A social prescriber connects individuals with non-clinical community services — such as art classes, gardening, volunteering, or support groups — to address the social determinants of health. They empower individuals to improve their mental, physical, and emotional wellbeing by providing personalized, community-based support that complements traditional medical care.Social Prescribers can support community members who are experiencing: - loneliness and disconnection - mental health challenges - financial challenges - food security or housing problems - And much more! (Such as filling out forms and referrals to other services etc...)
